Petitioners should bring the following items with them:
- Address for home, work, and places frequented
- Age, height, weight, eye color, hair color, and distinguishing marks
- A copy of an Emergency Protective Order if one was issued
- Full name and dates of birth of any family members you want included
Home, work, cell phone, and pager numbers
- The name and address of the person from whom you are seeking protection
- Name, nicknames, and aliases
- A picture ID of yourself, which can be a driver's license, work identification tag, or military ID
- Recent photograph of that person
- Social Security Number
