Worker's Compensation Filing Procedures

Attention Contractors - Certificate of Workers' Compensation Insurance

Electronic filing of the Contractor Certification Form 61A is now available. Complete the form and receive your acknowledgement of compliance.  

Follow these easy steps:
  1. You will need your policy information/declaration page with the effective and expiration date of your policy to complete this form.
  2. Visit their website.
  3. Complete the form fully and with care.
  4. Submit the electronic form.
  5. A certificate will be sent to you immediately to acknowledge compliance (see example (PDF)). Be sure to save, print, or download a copy for your records.
  6. Include a copy with your annual business license renewal, or application as evidence of compliance. Your Certificate of Business License will not be issued without this document.
  7. Paper filings will be accepted; however your acknowledgment will be mailed to you in approximately 2 weeks upon their receipt, which will result in a delay in our office issuing a Certificate of Business License.
