Family Assessment &
Preservation Services
The County of York and
the City of Poquoson are
providing family assessment and preservation services with at risk families.
Main Goals:
- Reduce potential for adolescents to be involved with
juvenile court.
- Reduce potential of out-of-home placement of
children.
- Reduce child abuse and neglect through parent
education and family therapy.
Programs & Services:
- Intensive Family Therapy which includes In-Home
services.
- Enhanced Case Management in collaboration with
schools, juvenile court, mental health programs, social services, recreational, etc.
Population Served:
- Adolescents, ages 11-18, who are beyond parental
control.
- Teens at risk of running away or at risk of
out-of-home placement.
- All families of those teens identified.
Referral Process:
To refer an adolescent or a family to this
service, the family and/or adolescent must be case managed by one of the agencies
mentioned above such as: Social Services, Juvenile Court, York or Poquoson School, Mental
Health Services, etc. The case manager will call the Family Preservation Worker to
determine if the referral is appropriate. After it has been determined that the case meets
the criteria, the family will then be referred by the case manager to the Family
Assessment and Preservation Team (FAPT) on the regularly scheduled monthly meeting.
