The York-Poquoson Commonwealth's Attorney and the Attorney General of Virginia in conjunction with Virginia TRIAD's are committed to reducing the victimization of seniors throughout the Commonwealth.  A major purpose of TRIAD is to develop, expand, and implement effective crime prevention and education programs for older Virginians.  They have put out a survey to provide them with feedback and information on the concerns of senior citizens.

You can participate in this survey three ways:

1. You can download the survey.
   Print the survey out, complete it, and mail it to the address at the end of the form.

2. Send us an e-mail requesting a printed copy of the survey, and one will be mailed to you.

3. Stop by the Senior Center of York, at 5314 George Washington Memorial Highway, Yorktown, VA, to obtain a printed copy of the survey.
